Return to the Pitch: A Systematic Review of Rehabilitation Protocols After ACL Reconstruction in Professional Football Players

Background:
Anterior cruciate ligament reconstruction (ACLR) is a common and career-defining procedure in professional football. Despite advancements in surgical techniques, intense debate persists regarding the optimal rehabilitation protocols and criteria for a safe return to play (RTP).
Objective:
To systematically review rehabilitation protocols, functional performance outcomes, and RTP timelines following ACLR exclusively in professional male football players.
Methods:
This systematic review followed the PRISMA 2020 guidelines. The search was conducted by one investigator, across PubMed, Scopus and Web of Science s and independently checked for eligibility and data extraction accuracy. Eligibility was strictly defined using the PICOS framework, focusing on professional male football players older than 16 years old. We evaluated studies detailing post-ACLR rehabilitation, objective functional performance, and RTP timelines.
Results:
Fourteen studies met the inclusion criteria, encompassing a total of 991 professional male players. We identified marked heterogeneity in rehabilitation approaches: 50% of the studies utilized non-detailed or club-specific protocols, while others employed accelerated (21.4%) or criterion-based (14.3%) approaches. Subjective clinical improvements plateaued between 6 and 12 months. Several studies reported persistent asymmetrical deficits, particularly in rate of force development and quadriceps strength, during the first months following return to play. RTP timelines ranged from 142.8 to 248 days, correlating strongly with the risks of secondary injuries.
Conclusion:
Clinical clearance and initial RTP success do not equate to full biomechanical restoration in elite football. The rehabilitation paradigm must transition from rigid, time-based progression toward strict, criterion-based protocols to minimize the alarmingly high rates of secondary ACL and thigh muscle injuries.
